DATA PROTECTION ACT 1998

Personal data will be processed in accordance with the provisions of the Data Protection Act 1998. Completed Incident Report Forms will be kept securely in the University Safety Office.

It is sometimes necessary for the University to permit the University Insurance Officer to see incident forms, and in turn they may provide copies, in accordance with the Data Protection Act 1998, to outside bodies in legitimate pursuance of legal proceeds.

The University Safety Office will also permit the University Occupational Health Unit to see incident reports when health issues may need to be followed up.

Incident Forms are also available to the relevant Safety Representative within the University to enable them to fulfil their functions in accordance with regulations concerning consultation with employees.

There is a legal requirement to notify certain types of incident to the Health and Safety Executive. In such cases the University Safety Office will provide information from the Incident Report Form on the prescribed notification form. The latter is copied to the Head of School and School/Divisional Safety Officer.
